The Vaccine Series Grid

The vaccine series grid begins on the second page and will display all vaccine groups, listed alphabetically. Within each group are dose cards for each dose in the series. For historical doses, each card will display the following information:

  • Dose number within the series
  • Date of administration.
  • Vaccine name. Note: this is the standard CDC name for the vaccine which may differ from the name displayed in the chart.
  • The source of the record.

Badge Color Coding

Consistent badging and color-coding are used throughout the report.

GREEN The vaccine series is complete.

BLUE For consideration at current visit: recommended or conditionally recommended.

TEAL Vaccine due at a future date.

GREY Vaccine is not indicated for the patient or the patient has aged out of the series.

Vaccine Series

The vaccine series grid is organized alphabetically by the vaccine group name. Doses within the series are listed in chronological order.

  1. Vaccine group.
  2. Badging: Color-coded badging, consistent with the Forecast Summary indicates the status of the vaccine series
  3. No Administrations. Any series for which no admninistrations were found will have this indicator.
  4. Dose number: The dose number within the series. For seasonal vaccines such as FLU and COVID-19, dose numbering resets each season.
  5. Extra dose: If a dose was administered outside of a series or after the series was completed, it does not count toward the series but is still considered valid. The dose number for extra doses will be displayed as EX.
  6. Future dose card. Any series which has a next dose in the future will have a future dose card with the earliest administration date.
  7. Due now card. Any series which has a dose due now will display a due now card.

CDC Hyperlink

Each series in the grid has a CDC hyperlink which will open a new browser window and navigate you to the CDC Immunization Schedule Notes specific for that vaccine series. This reference provides the most concise, easy-to-read guidance for each series including routine and catch-up guidance, special situations (high-risk indications), and contraindications and precautions. Invalid Doses

The most common reason for a dose being evaluated as invalid are doses given too young or doses administered below the minimum interval. These doses will be flagged with a strikethough on the date and a red invalid dose badge in place of the dose number. Because the most common cause of invalid doses are age and interval errors, the age at administration and dose interval are displayed at the bottom of the card.

  1. Invalid dose badge will appear in place of the dose number and the date will have a strikethrough.
  2. Evaluation reason(s). All of the reasons the Dose was evalauted as invalid are displayed after the vaccine name.
  3. Age at administration. The patient's age on the date of administration appears here. This element is only displayed on invalid doses.
  4. Administration interval. The interval since the previous dose is displayed for invalid doses where at least one dose was previously administered.

Live Vaccine Reminder

The live vaccine reminder appears in the Due Now card for the MMR and Varicella vaccine groups as a reminder to consider the 28-day live vaccine spacing requirements for these vaccines. The reminder is not displayed for FLU and H1N1 because the live intranasal versions of these vaccines are the least commonly administered vaccines for these groups.

Unsupported Vaccines
