• Single Hep B dose on chart, dose #2 forecast for 1 mo.
• RSV due now
• DTP, Hib, Pneumococcal, Polio, Rotavirus forecast for 2 mo.

Report for infant one day before first birthday with:

• Hep A, Hib, and Pneumococcal can be given (grace period).
• MMR #1 given too young, is invalid.
• MMR and Varicella re-forecast to later date due to invalid MMR and 28-day live vaccine spacing.

14 yr old
MMR incomplete

Report for 14 year old, with:

• Due for seasonal COVID-19 and Flu.
• Invalid 3rd Hep B dose but was re-vaccinated, series complete.
• Duplicate MMRV dose chart gives appearance of series complete if dates not checked. Patient is under-immunized. MMR dose 2 forecast and is due now.

3 yr old
Invalid Hep B, past due

Report for 3 year old, with:

• Due for seasonal COVID-19 and Flu.
• Invalid 3rd Hep B dose requires re-vaccination, is past due and patient is under-immunized.
